Shipping Site: If you ship out of multiple warehouses it is required to select which warehouse this order needs to ship from otherwise it will default to your main warehouse.

Requested Ship Date: The date you would like the order to be shipped

PO Number: internal use only ; not necessary

Services: Add an ice pack or additional insurance by selecting the boxes applicable
